I opened an account at a bank and now I am asked to fill out a form (IRS) for above. Looks straightforward but looks like I need to provide a US taxpayer identification number (since I cannot get Social Security # as I am ESL student).

Also, I'm not sure Taiwan has a income tax treaty with US.

Anybody have info on this form? Any info would be appreciated!

Technically it's illegal for banks to require a social security number for you to open an account, though many are in the habit of doing this anyway. You could ask them to generate a number for you, or choose to use another bank.

The form is likely among the many that you can download from the IRS web site. Just go to a search engine (www.metacrawler.com) and search with the words "IRS forms." Once you find the IRS forms site, scroll down until you find it listed. ('Should be there.)

Download it from there.
